Benton County Health Services – Primary Care

Welcom to Benton County Community Health Centers!  We are excited to become your health care provider.  Our Patient-Centered Primary Care Home (PCPCH) brings together a team of health professionals with one goal: keeping you healthy.  Teams are made up of doctors, nurses, behavioral specialists and others, working together to take care of you.

All four locations are accredited PCPCH by the State of Oregon at Tier 3.

Services

We offer interpretation services so you can receive care in your preferred language. Confidentiality is governed by state and federal law.  We provide the following services for all ages:

Women's health Annual exams Behavioral health

Chronic disease management Preventive services Laboratory services

Medical referrals Low cost medications Immunizations Case management services

 

We accept Oregon Health Plan, private insurance, and people without insurance. A sliding fee scale and payment plan is available.

Forms

If you are preparing for your first visit with us, please be sure to fill out the forms from the client registration and brochures forms page and bring them to your appointment. Filling out everything that you can ahead of time will save time for you and our staff.
