Community Health Centers - About Us

Mission

Community Health Centers of Benton and Linn Counties (CHCBLC) provides a medical home that promotes and supports health and wellness. We accomplish this by:

  • Bringing together medical, dental, mental health services and addiction services
  • Offering care that is inclusive and available to anyone regardless of insurance status, economic status, language, age or health status
  • Providing services at clinic locations in Benton and Linn Counties
  • Working together with patients/consumers/clients to have them involved in decisions and actions to improve their health
  • Coordinating community partnerships to provide a broader range of services than can be offered by the health center alone
  • Intentionally committing to being a leader in changing health care delivery by modeling an integrated health home that provides access to quality care

Who We Are

We have experienced and dedicated physicians and medical assistants on staff that include specialists in Family Medicine, Pediatrics and Internal Medicine. Our staff is committed to providing high-quality, patient-centered care. Dr. Charlene Clark, Internal Medicine Physician and Medical Director, leads a team of providers focused on improving the health of our patients. Our Board
We have a 13-member Board of Directors responsible for governance. Over half of the Board members are regular clients of the Health Centers. Community representatives make up the remainder of the Board.

Who We Serve

In 2008, we were a medical home for over 7,000 patients. During that time we experienced nearly 14,000 medical visits and over 20,000 mental health and substance abuse visits. This year promises to bring even more County residents to our Health Centers, who we serve regardless of income and insurance status. Read more about our services...

Funding and Costs

Pie Chart Showing Funding DistributionWe receive funding from several sources. A federal grant represents 22 percent of total revenue. FQHC status also assists in leveraging state and local resources. In 2007, the budget for the Health Center was $4,149,480 million. Read more...
