Vital Records

Vital records, and the statistics that can be generated from them, are an important tool in determining and reporting the health status of our community. They also play an important role for each person. People need certified copies of birth and death certificates to establish, among other things, their identity, social security benefits, and to apply for a driver's license, school entry/registration, or passport.

A certified copy of a vital record must contain the stamp or seal of either the local or state Registrar and must be printed on approved certificate paper. These steps are taken to prevent fraud.

Benton County can issue certified copies of birth or death certificates at the county level for six months after an event occurring within Benton County. To order certificates for events more than six months past, contact Oregon Vital Records, P.O. Box 14050, Portland, OR 97293 or telephone 1 (971) 673-1190. The costs are the same.

The hours for Vital Records are 8 a.m. to 12 p.m. and 1 p.m. to 5 p.m., Monday through Friday, with the exception oflegal holidays when the office is closed. Walk-in service is available at the Benton County Health Department, 530 NW 27th St., Corvallis. Please inquire at the main reception desk, or for further information, call (541) 766-6622.
