CaCoon Program

The CaCoon Program focus is on the child, birth through age 20, with complex health conditions that results in the need for specialized medical, educational, vocational, and social services, as well as the impact of those needs on the family. The primary service of the program is care coordination.

CaCoon Program

Services provided are:

  • Family and child health assessment
  • Monitoring child health and development
  • Linking families to specialty health care and community resources
  • Support and counseling
  • Advocacy
  • Service coordination and community consultation

CaCoon services are provided primarily in the home, but may also occur at the Health Department, in the hospital or by phone.

Eligibility

CaCoon services are dependent on diagnosis and open to all income levels. Children who qualify for services include, but not limited to:

  • Children with congenital conditions such as spina bifida, cleft lip and/or palate and heart defects
  • Children with genetic disorders such as Down Syndrome, Fragile X and PKU
  • Children with conditions resulting from prematurity such as Cerebral Palsy, seizure disorders, and feeding disorders
  • Children and chronic conditions such as asthma and juvenile rheumatoid arthritis, autism and ADHD, hearing and vision impairments
